Short answer
Prepare the same UK permanent-import evidence: original Portuguese registration evidence, HMRC/NOVA confirmation, VAT/duty evidence if due, approval evidence and V55/5 support documents.
Official position
GOV.UK does not create a separate V55/5 route just because the vehicle comes from Portugal. It says permanent imports need HMRC/NOVA processing, any VAT or duty paid, proof of vehicle approval, and original imported-vehicle supporting documents.
What to do
Get the original Portuguese registration certificate or official registration evidence.
Keep purchase, collection, transport and customs evidence for HMRC and DVLA.
Check for CoC or approval evidence before filling technical V55/5 boxes.
Use a translation or covering note if the key identity and date evidence is not clear to DVLA.
