Box 70 of the DVLA V55/5 form is for keeper name & address. It collects 11 entries on the form: who will be the registered keeper?, title, first names (in full), surname, business or company name, dvla fleet number, address line 1, address line 2, address line 3, post town and postcode.
If the vehicle is registered in a business or association name, unincorporated bodies must also give the full name of the person responsible for the vehicle.
